But where would I enter those in, it seems like if I want to edit my macros, it only allows the ctrl or the ctrl + shift options

Basically, what I am trying to do is to set up a macro that uses the ctrl + shift + alt + arrow keys as the short cuts. I am not sure how to set the hotkeys other than going to Tools>Macro>Macros and then clicking options to change the hot keys. Let me know if you can help.

Unfortunately you cannot include the ALT keys in that dialog box.
You'd have to use VBA code to incorporate the ALT keys. That was what we actually posted for you in our original reply.

3) Create a separate macro with the unload code
4) Have the macro un-load once the add-in is unloaded

The actual code:
Application.OnKey "^+d", "DollarOne"
"^+d" is the shortcut you assigned
"DollarOne" is the name of the actual macro or routine to run

Code to unload the shortcuts:
Application.OnKey "^+d"
by leaving out the macro or routine at the end, it basically assigns the shortcut to do nothing
